LICENSE AGREEMENT

By Using The Enclosed Forms You Accept All The Terms And Conditions Of This License Agreement

License: InfoLink Screening Services, Inc. (herein "InfoLink") grants you a license to use the enclosed files (the "forms") in accordance with the terms of this license. The copyright and all other rights in the forms and the accompanying written materials shall remain with InfoLink with all rights reserved.

You May: (1) use the forms on any single computer, provided that the forms are in use on only one computer at a time. The forms are "in use" on a computer when it is loaded into the temporary memory (i.e. RAM) or installed into the permanent memory (i.e., hard disk) of that computer. (2) make a single copy of the forms solely for backup purposes. (3) modify, publish and distribute the photographic images except as set out below and in the caption text associated with each individual image. (4) permanently transfer your rights under this Agreement to a third party by providing to such party this Agreement, the forms, and all accompanying written materials, provided that you retain no copies of the forms or written materials and that the other party agrees to the terms of this License Agreement.

You May Not: (1) make (or allow anyone else to make) any form of copy, whether digital, printed, photographic or otherwise, of any of the images in this Product, except insofar as such images may be incorporated by you into on-screen or printed material (see captions of photos in the forms for any additional restrictions on usage of photos). (2) place (or allow anyone else to place) any of the images in this Product on an electronic bulletin board or other form of on-line service. (3) use (or allow anyone else to use) any of the images in this product in any form of pornography, or in any manner which is in any way defamatory of any person or business entity in any such image, or which implies any form of endorsement by InfoLink, its suppliers, its licensers, or any individual or entity depicted in these images. (4) reverse engineer, decompile or disassemble the Software. (5) modify, adapt, translate, rent, lease, loan or create derivative works from the forms. (6) export the forms in any form without the appropriate United States and foreign government licenses.

Term: This License shall continue for as long as you use the forms, except that it will terminate if you fail to comply with any term or condition of this Agreement. Upon such termination, or in order for you to terminate this Agreement, you agree to destroy the original and any copies of the forms. The Limitation of Warranties and Liability set out below shall continue in force even after any termination.

Warranty: InfoLink warrants to you that the medium on which the forms were recorded will be free from defects in materials and workmanship for a period of ninety (90 days from your date of receipt. If such a defect occurs, return it to the address below and InfoLink will replace it for free.

Limitation of Warranties and Liability: Except for the express warranty above, THE FORMS ARE PROVIDED "AS IS" WITHOUT WARRANTY OF ANY KIND. These documents are provided only as samples or examples and no intent is made by InfoLink, its licensers or its developers, directors, officers, employees or affiliates with regard to the practice of law or providing legal advise in any way. As laws vary from state to state, no warranty is made regarding the documents here-in with regard to their legality within any state. It is highly recommended by InfoLink that the user/purchaser of the enclosed software consult with their individual labor and employment legal advisors prior to adopting any of the forms or procedures herein. InfoLink and its licensers make no other representations or warranties as to the software, either express or implied, including but not limited to the implied warranties of merchantability and fitness for a particular purpose. InfoLink's aggregate liability, including that of InfoLink's dealer's, suppliers and licensers, shall not exceed the amount paid by you for the forms. In no event will InfoLink, its licensers or its developers, directors, officers, employees or affiliates be liable for any consequential, incidental or indirect damages (including but not limited to dames for loss of business profit, business interruption and loss of business information) arising from the use of or inability to use the software or accompanying written materials. Some states do not allow the exclusion or limitation of liability for consequential or incidental damages, so the above limitation may not apply to you.

Restricted Rights: Use, duplication or disclosure by the Government is subject to restrictions as set forth in subdivision (c)(1)(ii) of the Rights in Technical Data and Computer Software clause at DFARS 252.227-7013

General: You acknowledge that you have read this Agreement and understand that it is the complete and exclusive statement of your agreement with InfoLink Screening Services, Inc. This Agreement will be governed by the laws of the State of California.
